تطورت الجراحة في نهاية القرن الماضي فسارت باتجاهين معاكسين. فمن جهة تعالى سقف العمليات الجراحية الكبرى فوصل إلى زرع القلب والكبد وتبديل المفاصل وغيرها ومن ناحية أخرى تراجعت حدة وقساوة العمل الجراحي فأصبح ينفذ بثقوب صغيرة تزيل عنه صيته المعهود من الأوجاع والإنزعاج.
هذا الكتاب الشيق مخصص لهذه النزعة الثاية أتاح لي الزملاء الثلاثة أن أطلع على فقرات محتواه الكثيف بشغف وهو مكتوب بلغة عربية سهلة وسلسة باتجاه جمهور واسع غير طبي كما أنه مرجع مفيد للأطباء العامين غير المعنين بجراحة المنظار وبالوقت نفسه للجراحين القائمين بهذه الجراحة أو المواكبين لها.
لم يفت في الكتاب ذكر الفحص بالمنظار الذي بدا في أواسط القرن الماضي وكان يجري ببنج موضعي دون الدخول إلى المستشفى وكان من الممكن بواسطته تشخيص بعض الآفات في الكبد والذي لا يستحق أن يهمل كليا.
كما انني أثني على أهمية جراحة المنظار لأمراض الغدة فوق الكلية المسماة غدة الكظر (Adrenal gland) وكنا قبلها نقوم بجرح طويل يبلغ أحيانا عشرين سنتميترا لاستئصال غدة لا تتجاوز عادة ثلاثة سنتمترات تتبعها من وقت لآخر إشتراكات في موضع الجرح. أما اليوم فهي قابلة للإستئصال بثقوب صغيرة يمكن للمريض بعدها أن يغادر المستشفى في اليوم الثاني جراحة الكظر هي بنظري من أهم حسنات جراحة المنزار.
هذه الجراحة وهذا الكتاب هما مناسبة للتذكير بالمجهود الذي يقوم به الجسم الطبي بمعاونة الصناعيين لتخفيف عبء العمل الجراحي على المريض. وهو يتطلب من الطبيب أشهرا أو سنوات إضافية من التمرين والتخصص.
