لاحظ الباحثان من خلال المعايشة اليومية والمقابلات مع المنتسبين في المستشفيات المدروسة (إدارة، أطباء، فنيين) أن مستشفياتنا ومؤسساتنا الصحية ما تزال تعاني من عدم إستيعاب فهم التأثير والإنعكاس الإيجابي لأداة نشر وظيفة الجودة في تصميم الخدمات وتحسينها، ولا سيما في المجتمعات النامية التي لا تقارن فيها متطلبات الخدمة الصحية وضروراتها مع المجتمعات المتقدمة، إذ يتم إستخدام الأساليب المعتمدة في تطوير العلاقة بين المريض والمستشفى وتحسبنها بغية إيصال صوت المريض (متطلباته) إلى المستويات العليا في المستشفى معبراً عنها من جانب المريض بالمتطلبات ويتم ترجمتها إلى الواقع العملي بالإستراتيجيات والبرامج والسياسات وطرائق العمل بهدف الوصول إلى مستوىً أعلى من الرضا عن جودة الخدمات الصحية، للوصول إلى ذلك ينبغي تحقيق مواءمة ناجحة بين صوت المريض والمواصفات الصحية (الطبية والعلاجية) لدى المستشفى (إمكانيات المستشفى المتاحة).
وللوصول إلى هذا المستوى المتقدم يتحتم على المؤسسات الصحيّة تطبيق نظام نشر وظيفة الجودة (QFD) لما يشتمل عليه هذا البرنامج من آليات مناسبة لترجمة متطلبات الزبائن (المرضى) إلى مواصفات تستخدم في تكوين الخدمة النهائية، فضلاً عن قدرته على تحديد وتعريف موقع المستشفى بالنسبة لمنافسيها وصولاً إلى إمتلاك أسبقية تنافسية.
