أتي صدور هذا الكتاب بعد وقتٍ قصيرٍ جداً من صدور كتاب "مفهوم المواطنة" للكاتب نفسه، ويعدُّ امتداداً لمنهج مؤلفه نفسه، ولكن بتركيز التفكير في الوضع السوري، وقراءة الواقع في هذا البلد المتألم ومآلاته.
ويرى المؤلف أن التفكير في ضوء الألم يجعل صاحبه مؤهلاً ليكون طبيب نفسه، من طريق فهمِ الألم بصورةٍ أعمق، وفهم مسبباته تمهيداً لابتكار طرق وآلياتٍ جديدة من أجل حياة كريمة من دون ألم. فــــيقول إن "الألم سلبي، والتفكير في ضوء الألم إيجابي: الألم انتقام، والتفكير في ضوء الألم عدالة؛ والألم شقاقٌ، والتفكير في ضوء الألم اشتقاق؛ والألم حرب، والتفكير في ضوء الألم سلام، والألمُ ابتذال، والتفكير في ضوء الألم صيانة؛ والألم تيه والتفكير في ضوء الألم وطن".
واستناداً إلى ذهنية تشتق منهجياتها من مفهومات "العدالة" و"الاشتقاق" و"السلام" و"الصيانة"، يضع المؤلف مجموعةً من الدراسات تشكل بمجملها ما يسميه "مقدمات تفكيرٍ في ضوء الألم السوري"، ويقدمها إلى قارئه في قسمين وأربعة فصول، يتناول القسم الأول توصيف البنية السياسية للنظام والمعارضة، بوساطة نصين وضعهما الكاتب ليكونا مثالاً يؤدي إلى تكوينِ "انطباعٍ مقارب لذهنية النظام والمعارضة في سورية".
