تتمحور القضية المهمة في كتاب "المرض حتى الموت" حول سؤال جوهري: من هو الإنسان؟ويهدف إلى معالجة قضية اليأس والخطيئة والإيمان، وما تتجلى عنها من إشكالات وجودية وأخلاقية تواجه الإنسان في حياته، إذ لا يمكن فصل علم النفس عن الأنثربولوجيا وعلم الأخلاق. وعلى الرغم من أن الكتاب يتضمن عنوانا ثانويا"التهذيب والإحياء"، فإن كيركگورد أضاف عبارة "الإحياء" إلى العنوان في اللحظات الاخيرة قبل صدور الكتاب. وقد لمحّ بأن العنوان "تأملي للغاية أيضا". استعار كيركگورد فكرة العنوان من إنجيل يوحنّا، الفصل الثاني، الذي يتعلق بالعازر . الكتاب لا يتعامل مع قضية المرض الذي يؤدي الى موت الجسد، بل مع الموت الروحي والذاتي.وقد قدم كيركگورد في لغة كاشفة عمّا يحدث لنفوس البشر، مثلا، في النوادي الليلية، في ساحات المعارك، في العزلة، لكن ليس طبقا لهذه الأماكن وظروفها، بل بموجب تنوع اليأس الجوّاني الذي يصيب الناس في تلك الأماكن. يتكون الكتاب من جزأين: الجزء الأول سايكولوجي، والجزء الثاني عقائدي، ويتناول مجمل الكتاب بصورة رئيسية اليأس بأشكاله المتنوعة، إلى جانب موضوعة الخطيئة التي شرع في تناولها في كتابه "مفهوم الفزع".ظهرت فكرة "اليأس" منذ وقت مبكر في كتابات كيركگورد، فقد كتب في أوراقه عام 1636: "إن عصرنا هو عصر اليأس". كما تناول ذلك في كتابه " أما- أو". وفيما بعد قدم في كتابه «حاشية غير علمية ً ختامية» وصفا لأزمة دينية، ويستخدم حينها تعبير" هذا المرض ليس حتى الموت، إنه وصف الإضطراب أو عدم الإنسجام الموجود في الإنسان، الذي لا يعرف أو لا يريد أن يعرف علاقته الحقيقية بأناه الأبدية.
