تبدأ القصة بوفاة الطبيب "أندريه لويز" وانتقاله إلى عالم الروح، حيث يجد نفسه أولاً في منطقة مظلمة ومليئة بالمعاناة تُسمى "الأومبرال"، نتيجة لحياته التي غلب عليها الاعتزاز بالذات والإهمال الروحي. بعد فترة من التوسل والندم، يتم إنقاذه ونقله إلى مستعمرة روحية متطورة تُسمى "موطننا" (Nosso Lar)، وهناك يكتشف نظاماً اجتماعياً وعلمياً مذهلاً لم يتخيله كطبيب بشري. يبدأ أندريه رحلة شاقة من التعلم والعمل التطوعي في "غرف التصحيح" والمستشفيات، حيث يتخلى تدريجياً عن كبريائه المهني ليعمل كمساعد وممرض بسيط، ويتعلم أن الطب الروحي يعتمد على الطاقة، الفكر، والحب أكثر من الأدوية المادية.
تصل رحلته إلى ذروتها عندما يُسمح له بزيارة منزله على الأرض بعد سنوات، ليواجه الحقيقة المرة بأن حياته القديمة استمرت بدونه؛ فزوجته تزوجت من آخر، وبيته لم يعد يخصه. في هذه اللحظة الحاسمة، يمر أندريه بأكبر اختبار لروحه، فبدلاً من الاستسلام للغيرة أو الغضب، يختار تقديم المساعدة لزوج زوجته المريض ويستخدم قواه الروحية لشفائه. ينتهي الكتاب بتحول أندريه من "طبيب جسد" أناني إلى "خادم روح" مستنير، ويتم الاعتراف به رسمياً كمواطن في المدينة الروحية، مؤكداً برسارلته أن الموت ليس نهاية، بل هو بداية لرحلة تعلم لا تنتهي، وأن الحب غير المشروط هو المفتاح الوحيد للسلام في عالم الروح.
