في مستشفى الدكتور ترافرز، تقبع "كارول" مجنونة الطابق العلوي، كما يسميها جميع العاملين في مستشفى للمجانين، والتي بدأت للتو نوبة الصراخ المعتادة التي تدل على شيء من ألم وخوف.
في هذه الأثناء يبدأ عواء الكلب في خارج المستشفى، فينتبه الجميع أن أحد المجانين استطاع أن يفر، وبعد لحظات يعرف الدكتور ترافرز أنه قد وقع في مأزق كبير. فالمجنونة الهاربة هي الوحيدة التي لا يجوز لها أن تهرب !!، أما السبب فهو أن المجنونة التي فرت هي وريثة جون بلاندش /كارول (حفيدته) الغير شرعية والتي أوصى لها بثروة تعادل الستة ملايين دولار. "إنها ليست إبنة بلاندش، بل حفيدته لإبنته، وهي غير شرعية" أما إبنة بلاندش والدة كارول والتي اختطفت منذ عشرين سنة من قِبل لص محترف فلها حكاية أخرى"إن إبنة بلاندش اختطفها مجنون خطر فيه غريزة الوحش المفترس، فظلت بين يديه بضعة أشهر قبل أن يهتدي أبوها إلى مقرها وينقذها". ولكنها ما لبثت أن انتحرت بعدئذ بإلقاء نفسها من النافذة. ولكن الذي سوف نعرفه فيما بعد، هو أنها قبل أن تموت، ولدت طفلة وأن والد هذه الطفلة والتي هي "كارول / الحفيدة، هو اللص المحترف المجنون غريسون.
هذه الظروف التي عاشت فيها المجنونة الصغيرة جعلتها في حالة من الهدوء حيناً، ومن الجنون حيناً أخرى، ما استدعى حجرها في المستشفى، حتى قرر الأطباء أن "جنونها مرض عجيب من نوع إزدواج الشخصية ...". وعلى الرغم من جنونها وهدوئها أحياناً، فإن الجميع يبدأ بالبحث عنها، أما الغاية فهي الحصول على فدية تقدر ستة ملايين دولار.
أحداث شيّقة بانتظارنا، ومغامرات لا تخلو من عقبات، في رحلة البحث عن "مجنونة الطابق العلوي" والتي سوف تكون سبباً في موت الكثير من أبطال الرواية. أما هي فلا ...
