مصنف هذه المخطوطة هو أبو العباس أحمد بن عبد السلام الشريف الصقلي، كان مشهوراً في النصف الأخير من القرن الثامن الهجري وفي الربع الأول من القرن التاسع الهجري، ومن المشاع عنه أنه قد قرأ الطب في جامع الزيتونة في تونس، واشتغل في المارستان الذي أحدثه ملك تونس أبو فراس عبد العزيز للضعفاء والمساعين له من الكتب: كتاب حفظ الصحة، وكتاب شرح أرجوزة علي بن سينا في الطب، وكتاب الأدوية المفردة المعروف بمفردات الصقلي، وهو المخطوط الذي بين يدينا متنه محققاً.
وهو مشتمل على تسعة عشر باباً جمع فيها الأمراض من الرأس إلى القدم، إلا أنه لم يدرس المرض ولم يتعرض التيه لأسبابه وأعراضه وعلاماته، وإنما أخذ يذكر فيه اسم المرض في طالع المقالة ويكرر الأدوية بأنواعها، وقليلاً ما يورد وصفاً مفصلاً لها إلا أنه غالباً ما يحدد أسماءها المختلفة وإن كانت جبلية أو بستانية وكيفية استعمالها ومقدرا تناولها.
وهكذا جاء كتابه جامعاً للأدوية المختصة بعلاج الأمراض، كل مرض على حدة، فهو يذكر المرض في رأس المقالة ويعدّد الأدوية النافعة منه وكيفية استعمالها في المقالة، ومن الأدوية التي يذكرها بعضها نافع لأمراض الرأس، الخلق، التنفس، المعدة، الكلى، المثانة، الرحم، المفاصل، الحميات، القلب، الدورة الدموية، الظهر...
