هو السيد محمّد بن محمّد صادق بن محمّد مهدي بن إسماعيل بن صدر الدين (1258 ــ 1338 هـ)، (الذي سمّيت أُسرة آل الصدر باسمه) بن صالح بن محمّد بن إبراهيم شرف الدين (جدّ أُسرة آل شرف الدين)، وصولاً إلى موسى (أبو سبحة) بن إبراهيم المرتضى بن الإمام موسى بن جعفر (ع). ولدي في 17 ربيع الأول سنة 1362 هـ، في يوم المولد النبوي الشريف، الموافق 23 آذار سنة 1943 م في النجف الأشرف،حيث نشأ الشهيد الصدر الثاني في أسرة علمية معروفة بالتقوى والعلم والفضل، منهم: جده لأمه الشيخ محمد رضا آل ياسين (توفى 1370 هـ) وهو من المراجع المشهورين حيث زامنت فترة مرجعيته مع مرجعية السيد أبو الحسن الأصفهاني (توفى 1365 هـ)، ووالده العلامة حجةالإسلام السيد محمد صادق الصدر (توفى 1403 هـ).
