يناقش المؤلف مرض الطاعون والطرائق التي انتقل عبرها من الصين إلى أوروبا، واصفا الكوارث التي حلت بالمدن والبلدان التي أصابها والأعداد المفزعة للضحايا الذين قضوا بسبب جوائحه الثلاث الخطيرة. يقع الكتاب في سبعة فصول تتناول مسيرة الطاعون، وطرائق انتشاره، والآلام المبرحة التي تنزل بضحاياه ومن حولهم، والأعداد الكبيرة من الأرواح التي يحصدها بسرعة فائقة. كما يُلخِّص المؤلف تاريخ الطاعون في ثلاث جوائح طويلة، في كل منها سلسلة من الأوبئة المنفصلة. تبدأ كل جائحة بتفشي وباء أساسي في مدن عدة تفشيا متتابعا وسريعا، يتبعه ذيل من الأوبئة تنتشر في مساحات تصغر بالتدريج حتى تختفي لمدة قرن أو أكثر، ثم تعود جائحة أخرى للظهور من جديد. كما يتناول المؤلف الجائحة الثانية في أوروبا، لاسيما الموت الأسود الذي مهد لها، ثم يصف الجائحة الثالثة قائلا إنها كانت أقل ضراوة ولم تغير الأوضاع السكانية والاقتصادية السائدة.
