في السلوكية التقليدية فالصرع فى صميمه توتر يعيشه الفرد فيسعى عبر المحاولة والخطأ إلى تجريب كثرة من المسالك حق يقع على سلوك يحقق خفض هذا التوتر فيكون التعزيز ويثبت هذا السلوك فى صورة تعلم سيان انتمى إلى الميكانيزمات الدفاعية إو إلى العراض المرضية. وأما عند الظاهريائيه فإن الصراع يصطبغ بالفلسفة الوجودية عند رولوماى وبالتصورات الجشطلتية عند بيرلز وبالمفاهيم الفرويدية عند روجرز. ومهما نباينت النظرة إلى الصراع فإنه يظل المحور الأساسى الذى تستند إليه كل التيارات العلاجية.
من هنا كان الباب الأول فى هذا الكتاب تناولا للصراع خاصة عند التحليل النفسى والسلوكية التقليدية أما الباب الثانى فيستند إلى نفس هذا المفهوم الدينامى ليبلغ بالمنهج الكلينيكى إلى فهم الشخصية. وأما الباب الثالث فيعرض التيارات العلاجي المختلفة والتى تقوم كلها على المقابلة الشخصية وتستند إلى مفهوم الدينامية ومع ذلك فئمة استثناء واحد وهو هذه لحركة التى ظهرت فى السنوات الخيرة والتى تسمى نفسها سلوكية جديدة أو تعديل السولك والتى ترفض مفهوم الصراع مقتصرة عل مفاهيم التشريط والتشريط المضاد والتى تعتبر الأمراض النفسية والعقلية مجرد أعراض ليس وراءها من صراعات إنما هى تشريطات خاطئة يلزم تصحيحها تشرطيات مضادة وريما كان يكتمل هذا الكتاب لو أتيح له باب عن الأمراض النفسية والعقلية ليس غير مدخل إلى الصحة النفسية. وإذا كان قد أتيح لنا مع الطبعة الثانية أن نضيف باباً ثالثاً فقد تتيح لنا الطبعة الثالثة مع ذكل أن نضيف باباً رابعا يتناول فى إيجار- إن يكن غير وصفى- الأمراض النفسية العقلية.
